[1] Take a new file of 300 pixels, 300 pixels, of resolution 72 dpi in the RGB mode.

[15] Take the Shape Tool select this one.
[16] Create a shape right click to get selection, then take the Rectangular Marquee tool use these settings.
[17] Now create a shape like me.
[18] Make a new layer fill with white color.
[19] Add the following layer styles.
[20] Duplicate a shape clear layer style take the move tool now place it like me.
[21] For this shape add the following layer styles.
