[1] Take a new file of 500 pixels, 500 pixels, of resolution 72 dpi in the RGB mode.
[2] Start by creating a rectangle using rectangular shape tool, color is #d4d7de.
[3] Create a rectangle on the top using rectangular shape tool. Press Ctrl+T. Right Click and select perspective. Adjust the nodes to get the shape as shown.
[4] Add the following layer styles.
[5] Duplicate that layer. Press Cntl+T. Right Click and select Flip Horizontal.
[6] Create a rectangle on the top using rectangular shape tool. Press Ctrl+T. Right Click and select perspective. Adjust the nodes to get the shape as shown.
[7] Double click the layer to open layer style window. Apply settings as shown.
[8] Create a rounded rectangle on the top using Rounded rectangle shape tool.
[9] Subtract to shape area from the top bar. Create an Rectangle using Rectangle shape tool.
[10] Press Ctrl+T. Right Click and select perspective. Adjust the nodes to get the shape as shown.
[11] Double click the layer to open layer style window. Apply settings as shown.
[12] Add a logo.
[13] Create a shape with the help of Elliptical Marquee Tool, Go to Select>Modify>Feather radius 20px.
[14] Make a new layer and fill the selection.
