[16] Add the following layer styles.
[17] Duplicate that layer place it like me.
[18] Create a new shape with Rectangle Tool.
[19] Create a new shape with Rectangle Tool color is white.
[20] For this shape add the following layer styles.
[21] Create a small circle with Ellipse Tool.
[22] Add the following layer styles.
[23] Create a two shape with Rectangle Tool.
[24] Create the following selection with Elliptical Marquee Tool, Go to Select>Modify>Feather Radius 10px.
[25] Create a new layer above the background layer fill the selection with Black.
[26] Take the Text Tool create your text.
